Pfizer Inc. announced its next-generation pneumococcal vaccine candidate produced an immune response up to 15 times stronger than its current Prevnar 20 shot against a key disease-causing bacterial strain in a Phase 2 study.
"These Phase 2 results reinforce our confidence in a next-generation vaccine designed to expand protection across serotypes while improving responses to key residual disease drivers such as serotype 3," Annaliesa Anderson, Senior Vice President and Chief Vaccines Officer at Pfizer, said in a statement.
The study showed the 25-valent candidate, PF-07872412 (25vPnC), generated geometric mean titers for the critical serotype 3 that were 8.8-fold higher after the third dose and approximately 15-fold higher after the fourth dose compared to Prevnar 20. Serotype 3 is responsible for 15% to 20% of pneumococcal infections and is associated with high fatality rates, which can reach up to 47 percent.
The positive data de-risks a key pipeline asset for Pfizer, which is now advancing the 25-valent vaccine into a pivotal Phase 3 trial for infants that began in May 2026. For the adult market, Pfizer will pursue a 35-valent version, aiming to protect its multi-billion dollar Prevnar franchise against competitors like Merck.
The investigational vaccine adds five new serotypes to the 20 covered by Prevnar 20, potentially broadening protection to about 90% of disease-causing serotypes in children. The enhanced response against serotype 3 is a key goal, as existing vaccines have struggled to halt its community spread.
Based on the strong Phase 2 results, Pfizer is moving its pediatric program for 25vPnC into pivotal Phase 3 studies. However, for the adult population, the company has decided to leapfrog directly to an even broader 35-valent candidate (35vPnC). This adult vaccine candidate is expected to enter clinical development by the end of 2026, a strategy intended to maintain Pfizer's leadership in the adult market over the long term.
The successful development of 25vPnC and its successor could solidify Pfizer's dominance in the pneumococcal vaccine market for years to come. Investors will now watch for the commencement of the adult 35vPnC trial by year-end 2026 and subsequent Phase 3 pediatric data.
